After it happens, are you having everyone exit and resetting the instance?

We had one of our sorc healers swear he was blowing up the ball lightning immediately, so we watched him the next try and saw him go from 18k+ health with shield to 0. It was essentially a double explosion, so there is no way that would be considered a 'crit' IMHO.

After resetting the instance the two times it happened, we were able to go back in with no problems.
